Request a reference code for use with an online questionnaire.
Only Safety Coordinators may apply for a code. If you are not a Safety Coordinator, see your coordinator for a code.
Each location requires its own reference code. If you need more than one code, submit a request for each location.
Employees must enter their own location's reference code when completing the online questionnaire.
